LOST OR STOLEN PASSPORTS

Lost or stolen passports can be replaced at the Clerk of Courts office.

You are required to report a lost or stolen passport to the Department of State immediately. To report your lost or stolen passport, you must complete form DS-64 and mail it to the U.S. Department of State. The Department of State will send you a letter of acknowledgement verifying that you reported your passport lost or stolen.

If your passport has been lost or stolen, a replacement will not be issued; you must apply for a new one at any Clerk of Courts title office. When you apply, you will need to present your letter of acknowledgement along with all other necessary documents.
