CHANGING PASSPORT INFORMATION

To amend the information on your passport, complete form DS-5504 and mail it to the Charleston Passport Agency. This form covers changes such as name changes and alterations to descriptive data.  If your name has changed, you must submit your marriage certificate, divorce decree, adoption decree or court order along with form DS-5504.

No fee will be assessed unless an expedited passport is requested.
